Passports

Passport Renewal for Adults (ages 16 and older):

  1. Submit current or expiring passport
  2. Two recent 2x2" (5x5 cm) photos, see photo requirements
  3. If your present passport was valid for 10 years, complete form DS-82. If your present passport was only valid for five years, complete form DS-11(do not sign)
  4. USD 75 or USD 100 if use form DS-11

Note: Please print out and submit only one-sided pages of the application.
